Introduction: The aim of this study was to investigate the obstacles and facilitators of preventive strategies for medical errors in an educational hospital in order to help improve patient safety.
Methods: Error profiles of 1395, Error Reporting Process, and Error Handling. Then, with the interview from the patient safety team, significant obstacles and facilitators were identified. At the next stage, the self-written questionnaire was compiled according to the interview. After confirmation of validity and reliability, 240 questionnaires were distributed. 155 completed questionnaires were analyzed using SPSS software. The Cronbach's alpha coefficient was 0.891.
Results: The findings of this study showed that hospital personnel assessed the effect of process facilitators more than structural facilitators and the effect of process barriers more than structural barriers. Among the subsets of the structural facilitators of staffing personnel, among the sub-set of process facilitators, is the recruitment of a safety expert and among the sub-set of process facilitators a hidden culture and, finally, among the subsets of the process facilitators of the personnel The 24-hour coverage of the safety team has been evaluated more effectively.
Conclusion: According to the results of this study, in designing and implementing preventive strategies of medical errors, more attention is paid to hidden culture as a barrier and with the history of personnel as facilitators.
