fa تأثیر برنامه تحول سلامت بر شاخص‌های عملکردی بیمارستان‌های دانشگاه علوم پزشکی مشهد با استفاده از مدل پابن لاسو The Effect of Health Evolution Plan on the Performance Indicators in Hospitals of Mashhad University of Medical Sciences Using the Pabon Lasso Model تخصصي Special پژوهشي Research <div style="text-align: justify;">زمینه و هدف :بررسی میزان کارایی و اثربخشی، بخشی از اصلی ترین موضوعات در بیمارستان‌ها می باشد. چگونگی بهره برداری از تخت بیمارستانی در تعیین کارایی بیمارستان حائز اهمیت است. این پژوهش با هدف بررسی تاثیر برنامه تحول سلامت بر شاخص‌های عملکردی بیمارستان‌های دانشگاه علوم پزشکی مشهد انجام شده است. روش پژوهش: این مطالعه از نوع توصیفی تحلیلی است که به صورت مقطعی در 22 بیمارستان دانشگاه علوم پزشکی مشهد در سال 1392 و 1396 جهت بررسی تاثیر طرح تحول سلامت انجام شد. جامعه پژوهش، بیمارستان‌های دانشگاه علوم پزشکی مشهد (28 بیمارستان) بودند و نمونه پژوهش 22 بیمارستانی بود که در طول مدت زمان مطالعه فعال بوده اند. داده‌ها با مراجعه به واحد آمار بیمارستان‌های مورد مطالعه و استخراج شاخص‌های عملکردی جمع آوری گردید. تحلیل داده‌ها با استفاده از نمودار پابن لاسو در نرم افزار Excel  و همچنین برای تعیین رابطه از نرم افزار SPSS 16 و آزمون t زوجی استفاده شد. یافته‌ها: در سال 1392 و 1396 به ترتیب بیشترین درصد اشغال تخت 86/55 و 94/27 درصد بود. همچنین کمترین میزان آن به ترتیب 48/20 درصد و 54/21 درصد بوده است. بیشترین چرخش تخت قبل و بعد از تحول سلامت به ترتیب 144/28 و 184/97 بود و کمترین آن نیز 7/45 و 8/85 بوده است. همچنین بیشترین متوسط روز بستری بیماران در بیمارستان قبل از برنامه 40/10 و 6/33 روز و پس از برنامه 38/41 و 8/00 روز بود و کمترین این میزان قبل از برنامه 1/51 روز و پس از برنامه 1/41 روز بوده است. بین درصد اشغال تخت، متوسط روز بستری و چرخش تخت، قبل و بعد از اجرای برنامه اختلاف معنی‌داری مشاهده شد (0/05 > p). نتیجه گیری: پس از اجرای برنامه تحول سلامت تغییرات مطلوبی در موقعیت بیمارستان در نمودار پابن لاسو رخ داده است. امید است با استمرار و پایش منظم این برنامه شاخص‌های عملکردی بیمارستان‌ها بهبود یابند.</div> <div style="text-align: justify;">Background: Assessing the effectiveness and efficiency is one of the main issues in hospitals. Utilization of a hospital bed is important in determining the hospital's efficiency. The purpose of this study was to investigate the effect of Health Evolution plan on the performance indicators of hospitals affiliated to Mashhad University of Medical Sciences. Methods: This cross-sectional descriptive study was conducted in 22 hospitals of Mashhad University of Medical Sciences in 2013 and 2017 to investigate the effect of Health Evolution Plan. The study population included all hospitals (N = 28) affiliated to Mashhad University of Medical Sciences. The study samples consisted of 22 hospitals, which were active at the time of study.  The data were collected by referring to the statistical unit of the studied hospitals and extracting the functional indicators. Data analysis was performed using the Pabon Lasso chart in Excel software and SPSS 16. Furthermore, paired t-test was used to determine the relationship. Results: In 2013 and 2017, the highest bed occupancy rates were 86.55 and 94.27 %, respectively; whereas, the lowest rates were 48.20 and 54.21 %, respectively. The maximum numbers of bed turnover were 144.28 and 184.97 before and after the health evolution, respectively. However, the lowest rates were 7.45 and 8.85 before and after the health evolution, respectively. Moreover, the average daily hospital admission rates before the program were 40.10 and 6.33days in the studied years, respectively. After the plan, admission rates were 38.41 and 8.00 days in 2013 and 2017, respectively. The lowest admission rates before and after the plan were 1.51 and 1.41 days, respectively. A significant difference was observed between before and after implementation of plan regarding the percentage of bed occupancy, the average hospitalization period in days, and bed turnover (p < 0.05). Conclusion: After implementation of the Health Promotion Plan, favorable changes happened in the hospital status on the Pabon Lasso chart.
