fa ارزیابی عملکرد بخش‌های بستری بیمارستان‌های تحت پوشش دانشگاه علوم پزشکی بیرجند با استفاده از مدل پابن‌لاسو در سال‌های قبل و حین همه‌گیری کووید-19 Evaluating the Performance of Inpatient Wards in Hospitals Affiliated with Birjand University of Medical Sciences Using the Pabon-Lasso Model in the Years before and during the COVID-19 Pandemic عمومى General پژوهشي Research زمینه و هدف: ارزیابی عملکرد بیمارستان‌ها منجر به افزایش اثربخشی، کارآیی و ارتقای کیفیت خدمات ارائه‌شده و بهبود پاسخگویی به مشتریان می‌گردد. شاخص‌های کارآیی پرکاربردترین ابزار اندازه‌گیری عملکرد بیمارستان‌ها است. شیوع همه‌گیری کووید-۱۹، سیستم مراقبت‌های بهداشتی را در سطوح مختلف با پیامدهای مهمی بر سلامت، اقتصاد و ساختارهای اجتماعی تحت‌تأثیر قرار داده است. هدف پژوهش حاضر ارزیابی عملکرد بخش‌های بستری بیمارستان‌های تحت‌پوشش دانشگاه علوم پزشکی بیرجند با استفاده از مدل پابن‌لاسو در سال‌های قبل و حین همه‌گیری کووید-۱۹ بود. روش پژوهش: مطالعه حاضر به‌صورت مقطعی طی سال‌های قبل از همه‌گیری کووید-۱۹ (۱۳۹۸-۱۳۹۷) و حین همه‌گیری کووید-۱۹ (۱۴۰۰-۱۳۹۹) انجام شد. جامعه پژوهش شامل تمام بخش‌های بستری بیمارستان‌های‌ تحت‌پوشش دانشگاه علوم پزشکی بیرجند بود. عملکرد خدمات و کارایی آن‌ها با استفاده از فرم جمع‌آوری اطلاعات گردآوری و از طریق الگوی پابن لاسو، کارایی بیمارستان مورد تجزیه‌و‌تحلیل قرار‌گرفت. برای ترسیم نمودار پابن‌لاسو، توصیف داده‌ها و بررسی روند تغییرات، از نرم‌افزار Excel استفاده‌شد، بررسی عملکرد خدمات و روند تغییرات دوره قبل و حین همه‌گیری کووید-۱۹ با استفاده از نرم‌افزار SPSS ۲۲ و آزمون آماری مقایسه زوجی (paired sample t-test) انجام‌شد (معنی‌داری در سطح ۰/۰۵ تعیین گردید). یافته‌ها: یافته‌های مطالعه حاضر در سال‌های قبل و حین همه‌گیری کووید-۱۹ نشان‌‌داد، نسبت‌های عملکردی درصد اشغال تخت ۲۹/۲۶درصد کاهش، متوسط اقامت بیمار ۶/۹۰ درصد افزایش و میزان گردش تخت سالیانه ۳۲/۸۰درصد کاهش داشته‌است. نتایج آزمون آماری مقایسه زوجی با سطح معنی‌داری ۰/۰۵ نشان‌‌داد در بخش‌های داخلی و CCU (Coronary Care Unit) برای ۳ شاخص عملکردی درصد اشغال تخت، میزان گردش تخت و متوسط اقامت بیمار ارتباط معنی‌داری وجود داشت و در بخش‌ سوختگی ارتباط معنی‌داری وجود نداشت؛ در بررسی سایر بخش‌های جراحی کلیه و مجاری ادراری، جراحی گوش و حلق و بینی، ارتوپدی، روان‌پزشکی، اطفال، عفونی، جراحی عمومی، ICU (Intensive Care Unit) قلب باز، جراحی مغز و اعصاب و قلب و عروق، ۲ شاخص عملکردی درصد اشغال تخت و میزان گردش تخت، در بخش ICU، شاخص عملکردی درصد اشغال تخت و در بخش‌های نوزادان و NICU (Neonetal Intensive Care Unit)، شاخص متوسط مدت اقامت بیمار ارتباط معنی‌داری را نشان دادند. در بررسی کارایی بخش‌ها، تعداد بخش‌های ناحیه کارا از ۳۲/۱۴ به ۲۸/۵۷درصد و تعداد بخش‌های ناحیه ناکارا از ۲۵ به ۲۸/۵۷درصد تغییر یافت؛ همچنین تغییرات مثبتی در وضعیت کارایی بخش‌های ICU قلب باز و نوزادان و تغییرات منفی در وضعیت کارایی بخش‌های اطفال، داخلی، روان‌پزشکی و عفونی در بازه زمانی مورد نظر مشاهده شد. نتیجه‌گیری: با ورود همه‌گیری کووید-۱۹، عملکرد خدمات در شاخص‌های درصد اشغال تخت و میزان گردش تخت سالیانه و کارایی در بیمارستان‌های تحت‌پوشش دانشگاه علوم پزشکی بیرجند به‌دلیل محدودیت در ارائه خدمات به بیماران کاهش یافت. در طی سال‌های مورد مطالعه، به‌طور میانگین کمتر از یک‌سوم (۳۰/۳۶درصد) بخش‌های بیمارستانی در ناحیه کارا و درنتیجه دارای عملکرد مطلوب بودند؛ لذا ارزیابی مستمر دوره‌ای برای کمک به سیاستگذاران جهت بهره‌برداری از منابع در دسترس با استفاده از مدل پابن‌لاسو و یا دیگر مدل‌ها پیشنهاد می‌گردد.   Background: Evaluating the performance of hospitals will increase effectiveness, efficiency, and quality of services provided and improves responding to the customer. Performance indicators are the most widely used tools for measuring the performance of hospitals. The prevalence of Covid-19 pandemic has affected health care systems at various levels with significant implications for health, the economy, and social structures. Therefore, the present study evaluates the performance of inpatient wards of hospitals affiliated with South Khorasan University of Medical Sciences in the years before and during the COVID-19 pandemic. Methods: The current study was cross-sectional, which was conducted during the years before (2018-2019) and during the outbreak of COVID-19 (2020-2021). The research population included all inpatient departments of hospitals affiliated with Birjand University of Medical Sciences (28 departments). The performance of services was analyzed using data collection form, and through the Pabon Lasso model, the efficiency of the hospital was analyzed. Excel 2018 was used to draw Paben Lasso diagram, describe the data, and check the process of change. Service performance and change process before and during the COVID-19 pandemic were examined using SPSS22 software and Paired sample t-test statistical test (The significance level was determined at 0.05). Results: The results of this study showed that in the years before and during the COVID-19 pandemic, the functional ratios of bed occupancy percentage decreased by 29.20%, the average patient stay increased by 6.90%, and the annual bed turnover rate decreased by 32.80%. The results of paired sample t-test with a significance level of 0.05 showed that there was a significant relationship regarding the internal wards and CCU (Coronary Care Unit) between 3 functional indicators: bed occupancy percentage, bed turnover rate and average patient stay; there was no significant relationship in the burn ward. After examining the wards of kidney and urinary tract surgery, ear, nose and throat surgery, orthopedics, psychiatry, pediatrics, infectious diseases, general surgery, open heart ICU (Intensive Care Unit), neurosurgery, and cardiovascular surgery, 2 functional indicators of bed occupancy percentage and bed turnover rate, in the ICU ward, the functional index of the percentage of bed occupancy, and in the neonatal wards, the NICU (Neonetal Intensive Care Unit), the average length of patient stay showed a significant relationship. After examining the efficiency of the wards, the number of efficient districts changed from 32.14 to 28.57%, and the number of inefficient districts changed from 25 to 28.57%. Moreover, positive changes in the efficiency status of the open heart and neonatal ICU departments and negative changes in the efficiency status of the pediatric, internal, psychiatric, and infectious wards were observed in the target period. Conclusion: With the arrival of Covid-19 pandemic, service performance regarding the indicators of bed occupancy percentage, and annual bed turnover rate, and efficiency in hospitals affiliated with Birjand University of Medical Sciences decreased due to limitations in providing services to patients.
