fa مزایای اقتصادی تولید داخل داروهای بیوسیمیلار در ایران Economic Advantages of Domestic Production of Biosimilar Medicines in Iran تخصصي Special پژوهشي Research زمینه و هدف: بیوسیمیلارها را می‌توان نسخه ژنریک داروهای بیولوژیک برند در نظر گرفت. استفاده از بیوسیمیلارهای تولید داخل به‌جای داروهای بیولوژیک برند اصلی، یکی از راه‌های کاهش هزینه‌های دارویی است. علاوه بر کاهش هزینه‌های دارویی، تولید داخل داروهای بیوسیمیلار می‌تواند مزایای اقتصادی دیگری نیز به همراه داشته باشد، لذا در این مطالعه به بررسی مزایای اقتصادی تولید داروهای بیوسیمیلار در ایران پرداخته‌شده است. روش پژوهش: این پژوهش در سال 1399 در دو مرحله کیفی و کمی انجام شد. در گام اول، پس از بررسی متون، راهنمای پرسشگری طراحی شد و مصاحبه‌های نیمه ساختاریافته با خبرگان تا رسیدن به نقطه‌ی اشباع، پس از مصاحبه با 9 نفر از خبرگان حوزه‌های مختلف (واردات، تولید و سیاست‌گذاری) ادامه یافت. داده‌ها به روش تحلیل مضمون و با استفاده از نرم‌افزار MaxQDA 2020، کد‌گذاری و تحلیل گردید و مزایای اقتصادی تولید بیوسیمیلارها در ایران از دیدگاه خبرگان استخراج شد. در بخش کمی، مزیت قیمتی بیوسیمیلارهای تولید داخل به‌عنوان یکی از مهم‌ترین مزایای اقتصادی آن‌ها در مقایسه با معادل آن در کشورهای مرجع مورد تجزیه‌وتحلیل قرار گرفت. بعلاوه این مقایسه برای داروهای برند اصلی وارداتی هم انجام شد. یافته‌ها: از دیدگاه خبرگان، مهم‌ترین اثر اقتصادی تولید داخلی بیوسیمیلارها، کاهش هزینه‌های دارویی و هزینه‌های مرتبط با سلامت از طریق دسترسی به داروهای ارزان‌تر بود. سایر ابعاد اقتصادی این داروها، اشتغال متخصصان، توسعه فناوری‌های جدید، گسترش بازار دارویی، کاهش خروج ارز، افزایش استطاعت‌پذیری و دسترسی به دارو و درنهایت ارتقای سطح سلامت جامعه بیان گردید. در بخش کمی، پس از مقایسه قیمت 23 داروی بیوسیمیلار تولید داخل باقیمت بیوسیمیلارهای مشابه در کشورهای مرجع، قیمت 21 دارو کمتر از میانگین قیمت بیوسیمیلارهای مشابه و قیمت 18 دارو حتی کمتر از کمینه قیمت بیوسیمیلارهای مشابه در کشورهای مرجع بود. بعلاوه، تولید این داروها در کشور به‌صورت غیرمستقیم بر کاهش قیمت داروهای برند اصلی در کشور مؤثر می‌باشند به‌طوری‌که، در مورد داروهای بیولوژیک وارداتی نیز، از میان 28 دارو قیمت 26 دارو از میانگین کشورهای مرجع و قیمت 23 دارو حتی از کمینه کشورهای مرجع کمتر بود. نتیجه‌گیری: بر اساس یافته‌های مطالعه، نتیجه‌گیری شد که تولید داخلی داروهای بیوسیمیلار در ایران علاوه برافزایش دسترسی به داروهای بیولوژیک و ارتقای سلامت عمومی، مزایای اقتصادی متعددی ازجمله صرفه‌جویی در هزینه‌های دارویی را در پی خواهد داشت.   Background: Biosimilars can be considered generic versions of branded biologic medicines. Substituting branded biologic medicines with biosimilars is important to reduce medication costs. In addition to reducing drug costs, the domestic production of biosimilars can provide many other economic benefits. Therefore, this study aimed to investigate the economic benefits of producing biosimilar medicines in Iran. Methods: This study was done in 2021 through 2 qualitative and quantitative steps. In the first step, after literature review, the interview guide was designed and semi-structured interviews with experts continued until reaching the saturation point, after interviewing 9 experts from different fields (imports, production, and policy making). The data were coded and analyzed by thematic analysis using MaxQDA software and the economic benefits of producing the biosimilars in Iran were extracted. In the quantitative step, the price advantage of producing domestic biosimilars was analyzed as one of their most important economic advantages compared to its equivalent in reference countries. In addition, the same comparison was done for the main imported branded medicines. Results: From the experts’ viewpoints, the most important economic effect of the domestic production of biosimilars was the reduction of medication costs through access to lower-priced medicines. The employment of professionals, development of new technologies, expansion of the pharmaceutical market, the reduction of foreign currency outflows, increased affordability and access to drugs, and ultimately the improvement of the health level of the society were the other economic aspects of these medicines. In the quantitative step, after comparing the prices of 23 domestic biosimilars with the prices of the equivalent medicines in reference countries, the prices of 21 medicines were lower than the average prices of equivalent biosimilars and the prices of 18 medicines were even lower than the minimum price of the equivalent biosimilars in the reference countries. In addition, the production of these drugs in the country is indirectly effective in reducing the price of the main branded drugs in the country, so that in the case of imported branded biologic medicines, out of 28 medicines, the prices of 26 medicines were lower than the average prices in the reference countries and the prices of 23 medicines were even lower than the minimum of prices in the reference countries.
