fa آسیب‌‌شناسی بخش پژوهش نظام سلامت ایران با نگاهی به آینده Pathology of the Research Sector in Iran's Health System with a Focus on Future تخصصي Special پژوهشي Research زمینه و هدف: تقویت کارکردهای پژوهش سلامت و نگاه نظام‌مند به مسئله توسعه علم‌وفناوری برای ادامه مسیر و برون‌رفت از چالش‌های فعلی نظام علم‌وفناوری در بخش سلامت ضروری است. به همین علت آسیب‌شناسی وضعیت فعلی پژوهش از منظر کارکردی نقش مهمی در تبیین شکاف میان آنچه که هست و آنچه که باید باشد، ایفا می‌کند و در نتیجه روشن‌گر مسیر توسعه علمی بخش سلامت با علم بر چالش‌های فعلی خواهد بود.   روش پژوهش: در مطالعه حاضر از مصاحبه با خبرگان، مرور مستندات و تشکیل پانل خبرگی استفاده شده‌است. روش تحلیل داده‌ها، تحلیل کیفی با رویکرد تحلیل چارچوبی بود. مجموعا 32 خبره از روسای مراکز تحقیقاتی، صاحبنظران نظام ملی نوآوری، سیاست‌گذاری علم‌وفناوری، سیاست‌گذاری سلامت، دانشجویان تحصیلات تکمیلی و اعضا هیئت‌علمی، پژوهشگران ارشد، سازمان‌های مردم‌نهاد و متخصصین بین‌المللی به‌صورت هدفمند انتخاب شدند. مطالعه مروری بر اسناد فرادست مرتبط ملی و بین‌المللی در تقویت و تحلیل یافته‌ها انجام شده‌است. تحلیل محتوی یافته‌های دو فاز با رویکرد تحلیل چارچوبی انجام و در پانل خبرگی متشکل از نمایندگانی از مصاحبه‌شوندگان ارایه شد. نتایج پانل تحلیل و  برای اتخاذ رویکرد نگاه به آینده از ترسیم مثلث آینده برای به تصویر کشیدن کشش تصاویر آینده، وزن زمان گذشته و فشار زمان حال استفاده شده است.  یافته‌ها: یافته‌های مطالعه به‌دست آمده در چارچوب مثلث آینده شامل وزن زمان گذشته، فشار زمان حال و کشش تصاویر آینده تحلیل شده است. وزن زمان گذشته و فشار زمان حال در قالب چالش‌های گذشته و زمان حال نظام پژوهش سلامت در هفت کارکرد اصلی شناسایی و تحلیل شدند. کارکردهای هفت‌گانه شامل هدایت و رهبری، هماهنگی پژوهش و سیاست‌گذاری، ترجمان خط‌مشی‌ها و سیاست‌های اسناد بالادستی در سطح عملیاتی پژوهش، مدیریت منابع، ارزشیابی و پایش عملکرد، توانمندسازی نیروی انسانی و سیاست‌گذاری آگاه از شواهد، بودند. کشش تصاویر آینده شامل لزوم ترسیم تصویر کلان منسجم ملی با رویکرد آینده‌نگر، مشارکت‌گرا و شفاف است که در آن به مدیریت تضاد منافع، انسجام در مدیریت منابع و تقویت ارتباطات بین‌بخشی و میان‌بخشی توجه شود. <h1 dir="RTL" style="text-align:justify">نتیجه گیری: راهکارهای اصلی رفع چالش‌های فعلی نظام پژوهش سلامت کشور توافق بر مفاهیم، چشم‌انداز واحد و ثبات رویه‌ها و تقویت رابطه بین سطوح مختلف مدیریتی (فراوزارتی، وزارت و دانشگاهی)، تقویت آموزش و پژوهش در سطح وزات‌خانه و دانشگاه‌های علوم‌پزشکی، تقویت ساختار مناسب برای ترجمان دانش، تقویت صندوق‌های ملی حمایت از پژوهش‌ها، اصلاح نظام ارزشیابی و بازنگری اساسی در مدل توانمندسازی نیروی انسانی شناسایی شده است.  </h1> Background: Strengthening the functions of health research and taking a systematic approach to the development of science and technology are essential for overcoming current challenges in health sector. Therefore, the functional pathology of current research plays a crucial role in highlighting the gap between what is and what should be, thereby illuminating the path of scientific development in health sector with a focus on current challenges. Methods: In this study, interviews with experts, review of documents and conducting an expert panel were used. The method of data analysis is qualitative analysis with a framework analysis approach. A total of 32 experts were selected from heads of research centers, experts of the national innovation system, science and technology policy, health policy, graduate students and faculty members, senior researchers, NGOs and international experts. A review of relevant national and international documents has been done to strengthen and analyze the findings. The content analysis of the findings of two phases was carried out with a framework analysis approach and was presented in an expert panel consisting of representatives of the interviewees. The results of the panel were analyzed and to draw the future triangle consisting of the weight of the past, the push of the present and the pull of the future. Results: The findings of the study have been analyzed in the framework of the future triangle, including the weight of the history, the push of the present, and the pull of the future. The weight of the history and the push of the present were identified and analyzed in the form of past and present challenges of the health research system in seven main functions. Including stewardship and leadership, coordination of research and policymaking, translation of policies and upstream documents at the operational level of research, resource management, performance evaluation and monitoring, human resource empowerment, and evidence-informed policymaking. The pull of future includes the need to draw a coherent national macro image with a forward-looking, participatory and transparent approach, in which attention is paid to the management of conflict of interests, coherence in resource management and strengthening of intersectoral collaborations.
