fa زیرساخت‌ها و اقدامات لازم به موازات اصلاح تعرفه‌های خدمات پزشکی برای بهبود عملکرد نظام سلامت ایران؛ یک مطالعه کیفی تخصصي Special پژوهشي Research <strong><span style="font-family:b nazanin;">زمینه و هدف</span></strong><strong><span style="font-family:b nazanin;">:</span></strong> <span style="font-family:b nazanin;">شاید مهمترین چالش موجود در ارائه خدمات سلامت در ایران، موضوع تعرفه‌ها باشد. نقش وزارت بهداشت، درمان و آموزش پزشکی، شورای عالی بیمه همگانی خدمات سلامت وزارت تعاون، کار و امور اجتماعی و سازمان نظام پزشکی در تعیین تعرفه‌ها از نظر قانون مشخص است اما در اجرا با مشکلات جدی مواجه است<strong>. </strong>هدف از این پژوهش، شناسایی و معرفی اصلاحات مورد نیاز به موازات اصلاح تعرفه خدمات پزشکی و‌‌یا زیرساخت‌های لازم در این زمینه برای بهبود عملکرد نظام سلامت می باشد.</span><br> <strong><span style="font-family:b nazanin;">روش پژوهش: </span></strong><span style="font-family:b nazanin;">این مطالعه از نوع کیفی می باشد. نمونه گیری به روش هدفمند انجام شده و تا اشباع داده‌ها ادامه‌‌ یافت. مشارکت کنندگان 12 نفر از مدیران سلامت شهرهای تهران، اصفهان و تبریز انتخاب شدند که در راستای هدف پژوهش به مصاحبه‌‌ی عمیق با آنان پرداخته شد. روش تحلیل داده‌ها در این پژوهش بر مبنای روش تحلیل موضوعی است. معیارهایی مانند &laquo;موثق بودن اطلاعات&raquo;، &laquo;قابل اعتماد بودن&raquo;، &laquo;قابل اطمینان بودن&raquo; و &laquo;قابل تثبیت بودن&raquo; داده‌ها در نظر گرفته شد.</span><br> <strong><span style="font-family:b nazanin;">یافته‌ها: </span></strong><span style="font-family:b nazanin;">ی</span><span style="font-family:b nazanin;">افته‌ها نشان داد که زیرساخت‌ها و اصلاحات براساس نظر مشارکت کنندگان به 3 بخش؛ اول<strong>،</strong> اصلاحات مربوط به وزارت بهداشت که شامل اصلاحات ساختاری، اصلاحات مربوط به ارائه دهنده و پایه گذاری اطلاع رسانی صحیح است. دوم اصلاحات مربوط به بیمه سلامت که شامل اصلاح ساختاری و اقدامی که باید توسط دولت در مورد بیمه صورت پذیرد و سوم، اصلاحات لازم توسط دولت می باشد.</span><br> <strong><span style="font-family:b nazanin;">نتیجه گیری: </span></strong><span style="font-family:b nazanin;">به نظر می رسد علاوه بر پرداختن به مباحث و ابعاد مختلف مربوط به تعیین تعرفه بایستی مقدماتی پیش از اصلاح تعرفه و ‌‌یا اقدامی به موازات اصلاح تعرفه فراهم گردد تا جریان و فرایند اصلاح تعرفه نیز با موفقیت انجام گیرد. این اقدام و اصلاحات در رابطه با وزارت بهداشت، بیمه‌ها و دولت می باشد.</span> <strong>Background</strong><strong>:</strong> Perhaps the most important challenge in provision of health services is the issue of tariffs. The roles of the Ministry of Health, Treatment, and Medical Education, the Supreme Council of public health services insurance, Ministry of Cooperatives, Labor, and Social Affairs, as well as the Medical Council are specified in law. But it is faced with serious problems in implementation. The purpose of this study was to identify and introduce infrastructures and necessary actions parallel with reforms of medical service tariffs to improve health system performance in Iran.<br> <strong>Methods:</strong> Purposeful sampling was used and continued until data saturation in this qualitative study. Participants included 12 Health Managers of Tehran, Isfahan, and Tabriz, who were selected based on the study goals. These people were investigated with deep interview. Data analysis was based on a thematic analysis. Criteria such as &quot;credibility&quot;, &quot;reliability&quot;, &quot;validity&quot;, and &quot;stability&quot; of data were considered.<br> <strong>Results:</strong> Findings showed that the infrastructure and reforms, according to participants&#39; viewpoints were divided into 3 parts: first, the reform of the Ministry of Health, including structural reforms, reforms related to the provider, and establishment of correct informing; second, health insurance reforms, consisting of structural reforms and actions that should be taken by the government about the insurance; and third, the necessary reforms by the government.<br> <strong>Conclusion:</strong> First, it seems that various issues and aspects related to tariff determination should be considered. Furthermore, some preliminaries should be provided before tariffs&#39; reformation or some actions should be taken in line with that for the success of tariff reformation process.
